Overview
Physical Therapist – General, Clearlake, California, starts August 24, 2026 for a 13-week travel assignment. Earn 2,077 to 2,154 per week with 40 guaranteed hours. Day shift, Monday through Friday, in a rural 25-bed, critical access facility. This is a travel, temporary assignment managed by Healthforce Healthcare LLC.

Responsibilities
- Plan and conduct PT evaluations to inform individualized treatment plans
- Provide therapy interventions using established protocols and techniques
- Collaborate with the interdisciplinary team and participate as a co-leader in patient groups
- Develop and communicate patient care and discharge plans; ensure timely documentation
- Supervise assigned assistants and ensure appropriate supervision of support staff
- Maintain accurate and complete clinical records and progress notes

Requirements
- Doctor of Physical Therapy (DPT) degree; licensure to practice physical therapy in the state of California (or eligible with foreign equivalent)
- Active Physical Therapy licensure in state(s) of practice; BLS/CPR certification from an approved vendor
- Valid driver’s license; must be at least 21 years of age
- Knowledge of Home Health Care processes (HCHB and OASIS) preferred; 2 years of recent PT experience required
- Home health experience preferred (1 year); ability to work in a home-based or community-based care setting
- Must meet facility- or program-specific credentialing and safety requirements
