Responsibilities
- Perform initial and ongoing occupational therapy evaluations for residents to identify functional goals.
- Develop and implement individualized treatment plans to improve activities of daily living and functional mobility.
- Deliver evidence-based interventions and modify plans as residents progress toward goals.
- Maintain accurate documentation of assessments, progress notes, treatment plans, and discharge summaries.
- Collaborate with an interdisciplinary team to coordinate care, discharge planning, and transitions of care.
- Educate residents and families on therapy goals, home exercise programs, and adaptive strategies.

Requirements
- Active Occupational Therapist license ( Vermont or eligible to practice in the applicable state)
- Experience in geriatric care or skilled nursing facility settings preferred
- Strong evaluation, treatment planning, and documentation skills
- Ability to work 36 guaranteed hours per week within a 5x8 day shift (08:00–16:30)
- Willingness to participate in a travel, temporary assignment starting 08/03/2026 for ~13 weeks
