Responsibilities
- Conduct OT evaluations and develop individualized treatment plans for adults, with focus on activities of daily living, functional mobility, upper-extremity function, and cognitive-perceptual skills.
- Implement evidence-based OT interventions to promote independence and safe discharge planning; adjust plans based on patient progress and goal attainment.
- Collaborate with an interdisciplinary team to coordinate care, document changes, and communicate recommendations to patients, families, and care partners.
- Document patient progress and treatment plans in the electronic medical record (Epic experience may be required or preferred).
- Educate patients and caregivers on home care strategies, adaptive equipment, and safety to support transition to the home environment.
- Comply with licensure requirements, facility policies, and applicable state regulations; participate in ongoing quality and safety initiatives.

Requirements
- Active Occupational Therapy licensure in all applicable states of practice
- Master’s degree in Occupational Therapy or Doctor of Occupational Therapy (OTD): Required; only if obtained license on or after January 2007; or foreign equivalent
- CPR certification or Basic Life Support (BLS, HS-BLS, or RQI/BLS) from an approved vendor per policy
- Valid Driver’s License; must be at least 21 years of age
- Epic EMR experience preferred; willing to indicate if candidate has Epic experience
- Ability to perform home health duties and travel between patient homes as part of the assignment
- Start date 05/26/2026; 13-week contract
