Responsibilities
- Conduct occupational therapy evaluations for adult patients receiving home-based care
- Develop and implement individualized treatment plans to improve activities of daily living, mobility, and functional skills
- Deliver therapeutic interventions to promote independence; adapt tasks and environments to optimize safety and participation
- Educate patients and family caregivers on safety, energy conservation, activity modification, and fall prevention
- Monitor progress, adjust treatment plans as needed, and document outcomes in accordance with agency standards
- Collaborate with an interdisciplinary team to coordinate care and align goals with medical and social work plans

Requirements
- Active Occupational Therapist license in the United States or eligibility for licensure in South Dakota
- Ability to work in a home healthcare setting and participate in a travel assignment
- Day shift availability in Aberdeen, SD
- Strong documentation and communication skills
- Ability to work with adult patients in a home-based setting
- 13-week assignment with guaranteed hours and travel requirements
