Responsibilities
- Provide comprehensive occupational therapy evaluation and ongoing treatment for home-based patients with acute care needs.
- Develop and implement individualized treatment plans focusing on activities of daily living, mobility, cognition, and safety in the home environment.
- Reassess patients regularly, document progress, and adjust goals to meet clinical and payer- or regulation-based requirements.
- Educate patients and families on adaptive equipment, energy conservation, fall prevention, and home safety strategies.
- Collaborate with an interdisciplinary team to coordinate care and support transitions or discharges from the episode of care.

- Availability for a 13-week travel assignment starting May 18, 2026.
- Ability to work a day shift in a home healthcare setting.
- Strong clinical assessment, documentation, and communication skills.
- Willingness to travel to the Aberdeen, SD area and provide in-home occupational therapy services.
