About the Role

Headhunting Inc is currently recruiting Occupational Therapists to join a passionate and highly specialized interdisciplinary team delivering an innovative and integrated model of community care. What You’ll Be Doing

As an OT on this integrated care team, you’ll:

  • Conduct in-home assessments to evaluate cognitive, functional, and psychosocial abilities.

  • Collaborate with an interdisciplinary team of physicians, nurses, PTs, SLPs, and social workers to develop personalized care plans tailored to patients’ unique goals.

  • Recommend, source, and adapt assistive technologies to support patient safety and independence.

  • Provide education to caregivers on personal care, safe transfer techniques, and home safety best practices.

  • Maintain accurate clinical records, including assessments, care plans, progress reports, and discharge documentation.

  • Participate in team huddles and case conferences to coordinate and align patient care strategies.

Qualifications You Bring

  • Master’s Degree in Occupational Therapy

  • Current registration in good standing with the College of Occupational Therapists of Ontario (COTO)

  • 3–5 years of clinical experience in occupational therapy (preferred)

  • Proficiency in assessment, goal setting, and evidence-based treatment planning

  • Knowledge of assistive devices, adaptive equipment, and environmental modifications

  • Excellent communication skills and ability to work as part of an integrated care team

  • Reliable vehicle and valid Ontario driver’s license
