Overview
Licensed Practical Nurse (LPN) needed for a travel assignment in Utica, NY, at a skilled nursing facility. Start date: 09/01/2026 for a 13-week contract with a weekly pay range of $1,485 to $1,557 and 37.5 guaranteed hours per week. Shift is nights (10:30 PM–6:30 AM) on a 40-bed LTC unit. Local rate available for eligible candidates within 75 miles. This travel - temporary assignment requires NY LPN licensure with LTC experience and proficiency in PointClickCare.

Responsibilities
- Provide licensed nursing care to residents on the LTC unit, including assessments, monitoring, and interventions per the care plan
- Administer prescribed medications and document administration in PointClickCare EMR
- Observe, chart, and report changes in patient condition to the supervising RN; respond to urgent situations per facility protocol
- Support activities of daily living, safety precautions, and appropriate patient positioning; assist with mobility and transfers as trained
- Collaborate with the interdisciplinary team; participate in care planning and care plan updates; float within the same building as needed
- Maintain accuracy and timeliness of charting, ensure a clean, safe, and respectful environment for residents

Requirements
- Active NY LPN license; Nursys license verification for active licenses
- Current BLS certification
- 1+ year of experience in a LTC facility required
- EMR experience with PointClickCare
- Drivers license with address that matches demographics
- RTO (time off) requests must be included at time of submission
- Local rate available for candidates within 75 miles; willingness to travel to Utica, NY for the assignment
