Overview
Licensed Practical Nurse (LPN) – LPN specialty in a skilled nursing setting located in Utica, NY. Start date: 06/23/2026. Duration: 13 weeks. Weekly pay: $1,475–$1,521 per week. Guaranteed hours: 37.5 per week. Shift: 8-hour evenings (2:30 p.m. – 10:30 p.m.). This is a travel - temporary assignment in a skilled nursing facility environment.

Responsibilities
- Provide direct nursing care to residents in a long-term care setting according to individualized care plans
- Administer prescribed medications, monitor for adverse effects, and document actions; escalate concerns to the supervising RN
- Conduct routine assessments, monitor vital signs, and participate in care planning; support activities of daily living (ADLs)
- Assist with mobility, transfers, hygiene, nutrition, and memory care needs as appropriate for dementia populations
- Maintain accurate, timely documentation in PointClick Care EMR; communicate changes to the care team
- Float to all units within the building as assigned; respond to patient needs and maintain a safe, clean environment
- Adhere to infection prevention, safety, and quality standards; implement falls prevention and safety protocols

Requirements
- Active New York LPN license
- BLS certification
- 1+ year of LPN experience in a long-term care or skilled nursing setting
- Ability to work in a 40-bed unit with residents including MS, ortho, dementia, and memory care populations
- Proficiency with PointClick Care EMR
- Willingness to work evenings and to float within the facility; every other weekend required
