Responsibilities
- Provide direct patient care on the sub-acute rehab floor according to the plan of care developed with the interdisciplinary team
- Administer medications and monitor for adverse effects, documenting promptly in the EMAR system (PointClickCare)
- Assess and document patient status, report changes, and assist with care coordination and discharge planning
- Support activities of daily living, mobility, and comfort measures; monitor for safety and fall prevention
- Collaborate with licensed staff and other disciplines to implement care plans for residents with dementia, chronic diseases, and diabetes

Requirements
- Active New York LPN license
- BLS (American Heart Association) certification
- 1–3 years of practical nursing experience; SNF or sub-acute rehab experience preferred over LTC-only roles
- Ability to work on a sub-acute rehab floor in a long-term care setting
- Proficiency with EMAR systems; PointClickCare experience is preferred
- Willingness to meet a 100-mile radius rule for this assignment
