Responsibilities

• Provide direct nursing care to residents under the supervision of licensed staff, following the plan of care.
• Administer medications and treatments as prescribed, documenting responses and changes.
• Monitor vital signs, assess patient status, and report significant findings to the supervising nurse.
• Assist residents with activities of daily living and mobility, promoting comfort, safety, and dignity.
• Maintain accurate, timely documentation in charts and electronic systems; communicate changes to the care team.
• Support infection control practices, safety protocols, and care coordination with RNs, physicians, and other staff.

Requirements

• Active LPN license.
• Ability to work in a long-term care setting and support resident needs.
• Strong communication and teamwork skills to collaborate with the care team.
• Availability to fulfill guaranteed 40 hours per week, including day and/or evening shifts.
• Commitment to resident-centered care and professional practice standards.
