Responsibilities
- Administer prescribed medications and document administration in accordance with policy
- Monitor residents’ status, take routine vital signs, and report changes to the supervising RN
- Assist with activities of daily living and provide comfort measures to residents
- Perform standard nursing procedures within scope and collaborate with the care team
- Maintain accurate clinical documentation in the electronic health record and contribute to care planning
- Ensure safety, infection control, and compliance with regulatory standards

Requirements
- Active Licensed Practical Nurse license (or eligible for travel assignment)
- Experience in a long-term care or skilled nursing setting preferred
- Ability to work the listed shift pattern (3x12-hour shifts) and maintain reliability
- Strong communication and teamwork skills
- Proficiency with basic computer tasks and resident documentation
- Willingness to comply with onboarding requirements and facility policies
