Responsibilities
- Provide direct nursing care to patients in the home setting, including assessments, administration of medications, wound care, and monitoring of vital signs.
- Develop, document, and reevaluate care plans; modify interventions based on patient and family response.
- Collaborate with the interdisciplinary team to coordinate home-based care and connect patients with community resources.
- Maintain accurate, confidential documentation and communicate clinical data in verbal and written form.
- Demonstrate independent clinical judgment and manage emergencies with composure; participate in on-call rotations if required.
- Travel to patient homes using a personal vehicle; support flexible scheduling to meet patient needs.

Requirements
- Active Massachusetts LPN license; must be uploaded with profile.
- Massachusetts Temporary Nursing Service Agency (Nursing Pool) license required for temporary staffing.
- American Heart Association or American Red Cross Basic Life Support (BLS) certification.
- Minimum 1 year of acute nursing care; hospital-based experience preferred.
- Home health experience required; proficiency with OASIS highly preferred; infusion therapy experience preferred.
- Valid driver's license and access to a reliable vehicle; traveler must have vehicle; ability to drive in inclement weather.
- Education: Completion of an approved Licensed Practical Nurse program (LPN diploma or equivalent).
