Responsibilities
- Provide direct nursing care to residents under supervision of an RN, including routine assessments and postoperative checks as assigned.
- Administer medications and treatments per physician orders and facility policy, documenting administration accurately.
- Monitor residents’ health status, respond to changes, and escalate concerns to the supervising RN promptly.
- Support activities of daily living, including personal care, mobility assistance, and patient safety measures.
- Maintain clear, compliant documentation in the electronic health record and ensure consistency of care across shifts.
- Collaborate with the care team to plan and implement individualized care plans, noting progress and any barriers to care.

Requirements
- Current LPN license for Pennsylvania (active or valid multi-state license as applicable)
- Ability to work in a long-term care setting and adapt to a dynamic resident population
- Strong communication and teamwork skills with the interdisciplinary care team
- Reliable, punctual, and able to work evenings on a 5x8 schedule
- Familiarity with basic nursing procedures, documentation, and resident safety protocols
