- Monitor residents’ status, assess vital signs, and report changes promptly
- Assist with activities of daily living and basic nursing care
- Document care provided and update charts accurately and timely
- Collaborate with the care team to implement and adjust care plans
- Maintain infection control practices and adhere to safety protocols

Requirements
- Active Licensed Practical Nurse license (state specifics verified as part of credentialing)
- Ability to work night shifts in a long-term care setting
- Strong communication and accurate documentation skills
