Responsibilities
- Provide direct nursing care to adults in a long-term care setting, including monitoring vital signs, medication administration per policy, and routine treatments.
- Assist residents with activities of daily living (ADLs) and mobility to promote comfort, safety, and independence.
- Collaborate with the interdisciplinary team to implement and adjust individualized care plans; communicate changes in patient status promptly.
- Document all patient care activities accurately in the electronic health record; ensure compliance with established policies and infection control practices.
- Monitor for safety concerns, prevent falls, manage basic wound care, and support palliative or comfort care as directed.
- Educate residents and families on care plans, medications, and procedure expectations; respond empathetically to questions and concerns.
Requirements
- Active practical nursing license in New Hampshire or eligibility to obtain licensure.
- Education: Nursing diploma or equivalent (LPN).
- Experience in long-term care or related settings preferred.
- Ability to work 40 hours per week; 5x8-day shift pattern; flexible to cover additional hours as needed.
- Strong communication, teamwork, and documentation skills; ability to work effectively within a multidisciplinary team.
- Demonstrated reliability, professional conduct, and commitment to resident-centered care.
- Physically capable of performing essential duties in a long-term care environment.
