Responsibilities

- Update and collaborate on care plans with the supervising clinician and families

Requirements
- Active LPN license in Pennsylvania
- 2+ years of home care experience
- Proficient in wound care
- LPN education from an accredited program
- Ability to work in a home healthcare setting and serve assigned ZIP codes 19104 and 19139
- Availability for 8:00am–4:30pm shift and 40 hours per week
