Overview
Licensed Practical Nurse (LPN) needed for a travel assignment in Berlin, Vermont. This LTC–nursing home setting starts 05/25/2026 for 6 weeks, with a 3x12-hour night shift. Responsibilities
- Provide direct nursing care to residents in a long-term care setting
- Administer medications and treatments according to physician orders; document care and outcomes
- Monitor residents’ condition, respond to changes, and escalate issues promptly
- Collaborate with the interdisciplinary care team and communicate with families as appropriate
- Ensure adherence to infection control policies, safety procedures, and accurate charting; work with dementia patients as needed

Requirements
- Active Vermont LPN license
- BLS certification through the American Heart Association
- Minimum 3 years of LPN experience; LTC experience required; prior travel experience preferred
- Willingness to float to other units as needed; comfortable caring for residents with dementia
- Ability to complete a 7-year background check (nationwide, county searches) with FACIS-3 and VCIC within required timeframes
- Reliable transportation required; public transportation is not accessible in the area
