Responsibilities
- Provide direct patient care to residents in a long-term care setting under supervision
- Administer prescribed medications and document administration per facility policy
- Monitor residents’ conditions, identify changes, and communicate with the supervising nurse and care team
- Assist with activities of daily living (ADLs) and support resident safety, comfort, and dignity
- Maintain accurate clinical documentation in the chart/EMR and ensure adherence to infection control and safety standards
- Collaborate with licensed staff to implement and adjust individualized care plans

Requirements
- Active Licensed Practical Nurse (LPN) license
- CPR/BLS certification
- Ability to work effectively in a skilled nursing/long-term care environment
- Prior clinical experience in long-term care or similar settings preferred
- Strong communication, teamwork, and assessment skills
- Flexibility to work a travel assignment with a 40-hour guarantee and a flexible shift
