Responsibilities
- Provide bedside nursing care for residents, including monitoring vital signs, medication administration, and treatment administration within scope of an LPN.
- Support activities of daily living and mobility, assisting with transfers, positioning, and personal care as needed.
- Observe resident conditions, document findings, and report changes to the supervising RN and interdisciplinary team.
- Administer prescribed medications and document administration accurately in the medical record.
- Participate in care planning and discharge planning discussions with the care team to promote resident-centered outcomes.
- Collaborate with licensed nurses, therapists, and other care staff to ensure safe, consistent, and quality care.

Requirements
- Active NC LPN license or compact license
- BLS certification
- 1+ year of geriatric LPN experience
- Ability to work in a long-term care and acute rehab setting
- Strong documentation skills and ability to work within a multidisciplinary team
- Availability to work evenings, 8-hour shifts, and meet the assignment schedule
