Overview
Licensed Practical Nurse needed for a travel assignment in Long-Term Care. Location: Keene, New Hampshire, United States. Start date: 06/01/2026. Duration: 5 contract weeks. Weekly pay: $1,104 to $1,179 per week. Guaranteed hours: 40 per week. Shift: 15:00–23:30. Setting: Skilled Nursing Facility providing long-term care. This is a travel - temporary assignment through Healthforce Healthcare LLC.

Responsibilities
- Provide licensed practical nursing care to residents per individualized care plans and physician orders
- Monitor residents’ status, document observations, administer medications and treatments as prescribed, and report changes promptly
- Assist residents with activities of daily living; promote dignity, independence, and quality of life
- Collect and document vital signs; assess for safety risks and respond to abnormal findings following established protocols
- Support the care team with wound care, dressing changes, and routine procedures; ensure accurate, complete charting and documentation
- Maintain infection control practices, adhere to safety policies, and participate in care plan updates and interdisciplinary rounds

Requirements
- Active Licensed Practical Nurse license in New Hampshire
- Current Basic Life Support (BLS) certification
- Prior experience in long-term care or skilled nursing settings is preferred
- Ability to work the 15:00–23:30 shift in a fast-paced LTC environment
- Strong assessment skills, attention to detail, and effective communication with residents, families, and the care team
- Willingness to commit to a 5-week travel assignment with guaranteed 40 hours per week
