Responsibilities
- Provide direct patient care to long-term care residents per the plan of care
- Monitor vital signs, administer medications as ordered, and report changes promptly
- Assist with activities of daily living and maintain patient safety and comfort
- Collaborate with an interprofessional team (RNs, CNAs, physicians) to coordinate care
- Maintain accurate documentation in the electronic medical record and adhere to policy
- Ensure infection control practices and safety standards are followed; escalate concerns as needed

Requirements
- Active LPN license in good standing; NH licensure or eligibility for assignment
- Prior experience in long-term care or skilled nursing settings preferred
- Ability to perform 12.5-hour shifts on a 3x12-hour schedule (06:45–19:15)
- Willingness to travel for a temporary assignment
- Proficiency with basic nursing documentation and electronic medical records
