Certified Occupational Therapy Assistant travel assignment in Lebanon, New Hampshire. Start date: 06/15/2026 for a 13-week contract in a skilled nursing facility. Responsibilities
- Provide occupational therapy services under supervision in accordance with the plan of care for residents in a skilled setting.
- Implement therapy programs to improve activities of daily living, mobility, strength, balance, and functional outcomes.
- Perform standardized assessments, document progress notes, and update the supervising OT on patient status.
- Adapt treatment plans based on patient response, ensuring safety, proper use of equipment, and goal attainment.
- Collaborate with the interdisciplinary team to coordinate care, communicate changes, and support discharge planning.
- Maintain accurate, timely documentation and uphold infection control and patient safety standards.

Requirements
- Current Certified Occupational Therapy Assistant credential with eligibility to practice in New Hampshire (or as required by state regulations).
- Availability for a 13-week travel assignment with 35 hours per week.
- Ability to work daytime hours: 08:00–16:30, in a skilled nursing facility setting.
- Strong communication and documentation skills; ability to collaborate with a multidisciplinary team.
- Commitment to professional standards, regulatory compliance, and resident-centered care.
