Certified Occupational Therapy Assistant | Acute Care | Laconia, NH

Healthforce Healthcare LLC is seeking a travel Certified Occupational Therapy Assistant (COTA) with an Acute Care specialty for a 13-week assignment in a Skilled Nursing Facility setting in Laconia, New Hampshire. Start date is 09/07/2026. Weekly pay ranges from $1,733 to $1,782 with guaranteed hours of 35 per week. The shift is 08:00 to 16:30, and the assignment is a Travel - Temporary position. This role focuses on providing rehabilitative therapy services to residents requiring occupational therapy, coordinating with the broader care team to support functional independence and safe discharge planning.

Responsibilities
- Provide occupational therapy evaluations, treatment plans, and ongoing therapeutic interventions for residents in a skilled nursing setting.
- Implement evidence-based OT treatments to improve independence in activities of daily living and functional tasks.
- Document therapy sessions, progress notes, and outcomes in the patient record per regulatory and facility standards.
- Communicate regularly with the supervising OT, care team, and family/cersonal representatives to coordinate care and discharge planning.
- Adapt environmental setup, adaptive equipment, and task modification to support safe performance goals.
- Contribute to interdisciplinary rounds and participate in care planning for residents requiring short- and long-term rehabilitation.

Requirements
- Current Certified Occupational Therapy Assistant credential and active license in New Hampshire, or eligibility to obtain per state requirements.
- Preferred experience in acute care or skilled nursing facility rehabilitation settings.
- Strong documentation skills and ability to collaborate with an interprofessional team.
- Ability to work 35 hours per week and commit to a 13-week assignment.
- Availability to start on or around 09/07/2026.
