Transportation Driver

SUMMARY: The Transportation Driver will provide the transportation of patients to and from hospitals, medical offices, dialysis centers and private residences in a safe, secure and professional manner.

ESSENTIAL DUTIES AND RESPONSIBILITIES:

• Transporting patients and clients utilizing the company vehicle in a safe and professional manner
• Assist patients and clients as needed to ensure safe transfer
• Effective communicate with facility regarding schedule progress and to receive additional instructions
• Understand how to operate GPS, Two-Way Radio and/or Cell Phone to navigate effectively
• Complete daily vehicle pre-trip and post-trip inspections, and maintain vehicle cleanliness
• Maintain accurate activity and vehicle report logs
• Other duties as assigned

QUALIFICATIONS: To perform this job successfully, an individual must be able to perform each essential duty satisfactorily. The requirements below are representative of the knowledge, skill, and/or ability required.

• High school diploma or equivalent preferred.
• Active Illinois Driver’s License required.
• Must possess the ability to communicate effectively, orally and in writing.

• Must be able to move intermittently throughout the workday.
• Must be able to push, pull move and/or lift a minimum of 25 pounds to a minimum height of 5 feet and be able to push, pull, move and/or carry such weight a minimum distance of 50 feet.
• May be necessary to assist in the evacuation of residents during emergency situations.
• The noise level in the work environment is usually moderate.
