Guided by our core values of Passion, Respect, and Excellence, the CNA Transportation Coordinator provides compassionate, hands-on resident care while coordinating medical appointments and transportation services to ensure residents receive timely access to healthcare.

Responsibilities for CNA Transportation Coordinator:

• Provide basic resident care under the direction and supervision of licensed nursing staff in a safe, compassionate, and respectful manner.
• Assist residents with activities of daily living, including feeding, bathing, dressing, grooming, mobility assistance, and changing linens.
• Provide supportive care to ensure residents' physical, emotional, and daily living needs are consistently met.
• Observe residents and promptly report changes in condition, behavior, or care needs to nursing staff.
• Maintain a clean, safe, and comfortable environment that promotes resident dignity and well-being.
• Support nursing staff and the interdisciplinary team in delivering high-quality, resident-centered care.
• Schedule and coordinate physician appointments and other medically necessary resident appointments.
• Arrange transportation for residents to and from medical appointments while ensuring schedules are accurate and communicated appropriately.
• Coordinate with transportation providers, families, and healthcare offices to facilitate seamless resident care.
• Maintain accurate records of appointments, transportation schedules, and related documentation.

Qualifications for CNA Transportation Coordinator:

• Must hold a current, active, and unencumbered Certified Nursing Assistant (CNA) license in the state of Rhode Island
• Valid Basic Life Support (BLS) / CPR certification required.
• Previous experience scheduling medical appointments and coordinating resident transportation is required.
• Ability to work effectively with multiple residents in a fast-paced, team-oriented healthcare environment.
• Demonstrated compassion, patience, and commitment to providing high-quality resident care.
• Strong communication, organizational, and scheduling skills.
• Ability to coordinate multiple appointments while maintaining accuracy and attention to detail.
