A Brief OverviewTransplant coordinator collaborates with an interdisciplinary transplant team to coordinator care for patients throughout the transplant process including phases of referral, evaluation for transplant, time awaiting transplant, and post transplant. Acts as a point of contact for families/caregivers and patients to educate, guide, and support them through their transplant journey. Self-motivation and development is essential to success in this role to remain current with regulatory guidelines to practice and identify/participate in process improvement initiatives to promote optimal patient care and outcomes.
