Transplant Nurse Coordinator - Kidney/Pancreas-(260001Y3)

A Brief Overview

Transplant coordinator collaborates with an interdisciplinary transplant team to coordinator care for patients throughout the transplant process including phases of referral, evaluation for transplant, time awaiting transplant, and post transplant. Acts as a point of contact for families/caregivers and patients to educate, guide, and support them through their transplant journey. Self-motivation and development is essential to success in this role to remain current with regulatory guidelines to practice and identify/participate in process improvement initiatives to promote optimal patient care and outcomes.

What You Will Do
  • Coordinates and collaborates with an interdisciplinary transplant team to provide patient care including medical assessments, preparation for transplant, and required documentation.
  • Takes responsibility for self-development to maintain knowledge and skills necessary to care for patients in all phases of the transplant process occurring in both the in-patient and out-patient setting.
  • Act as a point of contact and communicate with families and caregivers regularly to provide education and navigation assistance throughout the transplant process including preparation for transplant, recovery expectations, and importance of adherence to plan of care
  • Ensure compliance with national and local transplant guidelines, regulations, and standards of care and participate in quality improvement initiatives when appropriate.
  • Initiates and maintains positive relationships with patients/customers, referring physicians, and /or clinics, payers, and supporting transplant staff.
  • Displays commitment to the mission of the hospital and its values.
Additional Responsibilities
  • Performs other duties as assigned.
  • Complies with all policies and standards.
  • For specific duties and responsibilities, refer to documentation provided by the department during orientation.
  • Must abide by all requirements to safely and securely maintain Protected Health Information (PHI) for our patients. Annual training, the UH Code of Conduct and UH policies and procedures are in place to address appropriate use of PHI in the workplace.

Qualifications

Education
  • (ADN) Associate's Degree in Nursing (Required) or
  • (BSN) Bachelor's Degree in Nursing (Preferred)
Work Experience
  • 2+ years of nursing experience (Required) and
  • Transplant experience (Preferred) and
Knowledge, Skills, & Abilities
  • Computer skills (Required proficiency)
  • Microsoft Office (Required proficiency)
  • Electronic Medical Records (Required proficiency)
Licenses and Certifications
  • Registered Nurse (RN), Ohio and/or Multi State Compact License (Required Upon Hire) and
  • Basic Life Support (BLS) (Required)
