Job Title: Transitions Coordinator
Supervisor: Associate Head, Cooke Transitions
Regular Daily Schedule: 8:30 a.m. – 3:45 p.m.
Calendar to Follow: 10 Month Academic + 10 Days
Status: Exempt
Term: 10 Months

Summary of Duties and Responsibilities
The Transition Coordinator is responsible for the preparation of students for an enviable life plan beyond Cooke.  This includes developing for each student a strengths based plan which will lead to the students’ highest possible level of independence and meaningful employment.   The Transition Coordinator will be responsible to include families in the development of the plan for each child.  The Transition Coordinator will coordinate each child’s entrance into post-secondary placement, up to 18 months post-graduation.  The Transition Coordinator will assist students in their personal, social, and academic development consistent with the goals, values, and mission of Transitions.  Performance responsibilities include but are not limited to:
- Family/student transition planning, counseling, and advisement, as appropriate depending on family need and student age.  Support may include, but is not limited to, acquisition of outside agency services, crisis support services, guardianship; special needs trust, SSI, OPWDD eligibility, respite services, and post-secondary enrollment and placement.
- Participation/organization of monthly family support/transition services meetings, as well as other relevant family support services meeting facilitation.
- Consultation to academic, vocational and related service departments to ensure transition goals are considered within Cooke curriculum.
- Maintain and develop professional networks with agencies and resources to further support Cooke families.
- Participation in advisory boards and developmental disability council meetings to maintain knowledge of changing system and policies, city-wide and across specific boroughs.
- Coordination and implementation of transition curriculum for school-based classes.
- Teaching of transition classes at respective site.
- Collaborate with the counseling department to identify family support needs.
- Monitoring/supporting the transition of our graduates for 18 months after they leave Cooke.
Essential Functions
- Must be physically present from 8:30 am through 3:45 pm - no exceptions can be made
- Must be able to adhere to the Physical Requirements listed below - no exceptions can be made

Requirements
- Masters Degree
- Prior experience with Students with Disabilities preferred
- Valid and current LMSW or LCSW License to practice in New York State
- Experience working with young adults with disabilities, adult services, and/or transition planning preferred

Professional Development
All Cooke staff are responsible for developing a professional growth plan with their supervisor.  Staff training is an integral part of the Cooke School and as such is a requirement of the job.  All staff must at a minimum: attend all scheduled Staff Development activities throughout the year and participate in staff orientation scheduled for the last week in August.
