and Responsibilities

As our Transitional Care Nurse Practitioner or Physician Assistant, you will help adult patients in our outpatient clinic receive comprehensive primary care, including diagnosis and management of common illnesses.

In this role, you will assume clinical responsibility for tasks performed within your scope of practice, collaborating with a physician/medical director. You will take patient histories, perform physical examinations, order diagnostic tests, establish diagnoses, provide care and management for common illnesses, facilitate disease prevention, make appropriate referrals, and participate in quality improvement programs.

To be successful in this role, you must be a licensed Nurse Practitioner or Physician Assistant with expertise in adult internal medicine, demonstrate exceptional clinical proficiency in patient assessment and management, and possess strong collaborative skills for working within a multidisciplinary healthcare team.

  • 100% Outpatient clinic
  • Assessment, evaluation, diagnostic determination, on-going treatment and clinical management, care planning and modification, medications reconciliation, risk assessment, and, coordination of care with the Acute Care Providers, Transitional Team and Primary Care Physicians
  • Direct contact and consultation with patient family members, advocates, and others to ensure effective communication of patient status and care plans
  • Day-to-day patient care management activity that includes professional input from all members of the patient care team
  • Accurate and timely completion of all clinical and professional records and documentation to assure coordination of care
  • In collaboration with the physician/medical director, assumes clinical responsibility for tasks performed within the scope of practice
  • Makes appropriate referrals to specialists and community agencies
  • Participates in quality assurance and quality improvement program
  • Works within multidisciplinary team to develop plan for the patient and their family

Job Requirements

Required

  • NP or PA Master's Degree from accredited program
  • Active or eligible license in the state of Arizona
  • Hold and maintain national certification in NP/PA area, under AZ regulations
  • American Heart Association BCLS

Preferred

  • Three years experience in internal medicine
