The Transitional Care Coordinator I is knowledgeable about disease, treatment, available community and governmental resources, psychosocial implications for individuals and families as it pertains to their age and stage of development; provides appropriate interventions and discharges planning services to the patients and
families; and is pragmatic in approaching end of life and ethical issues. Communication is key to the role and various modes of communication are utilized. Performs all other duties assigned by the Transitional Care Manager and Case Management Director, utilizing the support of the Discharge Planning Lead.
Job Qualifications:
Required:
• Bachelor's degree in Sociology, Psychology, Human Services or related field
• Obtain a CPR license within 1 month of employment
Preferred:
• Bachelor’s degree or a master’s degree in social work
• 1 year of health care experience with discharge planning or care coordination experience
