Wildwood School, a part of Wildwood Programs, provides a comprehensive educational program for students with autism spectrum disorders, and developmental disabilities to prepare them to be as independent as possible; Academic, social, employment, and functional skills are taught to each student as driven by their Individual Educational Plan (IEP), needs, and interests.
Who are we looking for:
We are looking for a creative Transition Specialist to support the classroom teams in ensuring effective transition planning for all studentsand develop and maintain relationships with OPWDD, OMH, and ACCES-VR, and the providers of their services, in order to remain current on information relevant to transition.
Essential Duties and Responsibilities include the following. Other duties may be assigned.
Works collaboratively and holistically with students, families, staff, service providers and employers in ensuring the provision of transition services to individuals.
Assists teams in developing and implementing Measurable Post-Secondary Goals and the Coordinated Set of Transition Activities throughout the IEP.
Assures that teams provide avenues for parents and guardians to participate in their child's transition plan.
Acts as a resource to social workers, families and other team members in developing transition plans.
Assures classroom teams execute and complete transition goals and activities for students.
Develops partnerships with similar agencies with programs for people with disabilities.
Remains current on, OPWDD, ACCES-VR, OMH, Medicaid, SSI and other state and federal programs and related rules and regulations governing their delivery by reading, attending meetings or training sessions.
