Job Description

The Transition Specialist (TS) provides comprehensive, culturally competent services to individuals detained in city jails and supports their successful reentry into the community through the Pathway Home 6 program. This role includes assessment, planning, coordination, and delivery of evidence-based supports, crisis intervention, and collaboration with internal and external stakeholders to ensure continuity of care. This role ensures that each member meets a minimum of 90% key performance indicator (KPI) goals, complies with contractual obligations, and continuously improves the jobseeker experience and employment outcomes.

The TS collaborates with cross-functional teams (i.e., education, job placement, financial counseling), supervisor, and senior leadership to monitor and track performance and find solutions to maximize outcomes for participants. This is a hands-on, relationship-driven role that requires strong engagement skills, effective case management and wraparound services competencies, excellent organizational skills, and a commitment to providing excellence in service delivery.

Your Responsibilities Include:

Effective Engagement & Individualized Services
• Provide individualized services to 100% of Participants on caseload from point of orientation to exit.
• Deliver services using a variety of modes to appeal and cater to Participants’ distinct preferences and engagement styles.
• Determine appropriate job placements for ≥ 90% Participants on the employment track.
• Proactively evaluate the role that education, occupational training will play in improving Participants’ employability and ability to achieve better, longer-term stable employment.
• Ensure each employment opportunity incorporates ≥ 90% Participants’ needs and preferences and provides a pathway to family-sustainable employment.

Case Management & Wraparound Services
• Provide Wraparound Case Management services to 100% Participants, including but not limited to:
o Establishing rapport and trusting relationships with Participants.
o Interacting Participant minimum of 1X/month.
o Motivating, guiding, coaching, and counseling Participants.
o Assessing Participants’ needs, strengths, interests, aptitudes, experience, capabilities, career goals, and family situation.
o Monitoring Participants’ progress in enhancing the Member’s education and skills, and preparing for, finding, and retaining employment.
o Completing LSCMI Assessment and career readiness checklists
o Facilitating referrals and connections to other internal personnel or community-based providers.
o Documenting 100% case management activities in the Salesforce (SF) and FedcapCARES (FCC).
• Ensure ≤ 10% error rate in internal case record audits.

Data and Performance Management
• Ensure 90% of caseload achieves ≥ 90% of KPI Contract Requirements.
• Analyze caseload performance minimum of 2X/month in partnership with Supervisor to identify strengths, challenges, gaps, and improvement needs.
• Monitor, discuss, and track improvement needs minimum of 1X/month in partnership with Supervisor and co-develop action plan or performance improvement plan (PIP) accordingly.
• As directed by and in partnership with Supervisor, review, discuss, and address needs of action plan or PIP with 100% accuracy, including actions, follow-ups, owners, and outcomes.

You're a Great Fit for This Role If:


• You hold a bachelor’s degree from an accredited institution preferred; minimum of associate's degree or equivalent credential required.
You bring a minimum of 2 years of experience in career services, case management, workforce development, or social services OR in related fields, such as education, healthcare administration, retail or hospitality management, or human resources.
• You are able to pass security clearance with NYC Department of Corrections

Compensation:

$64,350-$65,000
