The Transition Services Liaison is responsible for ensuring communication and cooperation of students participating in a transition track and assisting students with the transition from high school to post- secondary education/training as appropriate.
License and Certification: State of Ohio Paraprofessional Certification
RESIDENCY REQUIREMENT Ohio resident preferred
Will consider applicants from a surrounding state to Ohio within one hundred miles from the Ohio border.
Border states include Michigan, Pennsylvania, West Virginia, Kentucky, and Indiana.
Applicants must be able to meet travel requirements and be willing and able to obtain Ohio Certification.

The Transition Services Liaison is responsible for ensuring communication and cooperation of students participating in a transition track and assisting students with the transition from high school to post- secondary education/training as appropriate.
ESSENTIAL FUNCTIONS:
Establishes positive relationships with agencies associated with family advocacy groups within an assigned geographic regionwith regard tocollege preparation, work study, and functional life skills;
Assists the Transition Coordinator with family assistance in the areas of college registration, Ohio Core requirements for graduation, work study programs, functional life skills and post-secondary options for students with disabilities;
Assists with and participates in the Individualized Education Plan (IEP) team;
Assists with post-secondary surveys and follow-up responses as required by the State Performance Plan;
Assists the Family Community Liaison in providing communication to assist families in maintaining advocacy and support for students with disabilities
Educate parents on outreach programs available in their geographic region as it relates to job searches, college admissions requirements, graduation requirements, functional levels as it relates to life skill areas, and other needs as indicated in the transition program for the student

MINIMUM REQUIRED QUALIFICATIONS
State of Ohio Paraprofessional Certification
Associate Degree in Social Services AND
1 or more years of experience in a customer service environment OR
Equivalent combination of education and experience
Microsoft Office (Outlook, Word, Excel, PowerPoint, Project, Visio, etc.); Web proficiency
The ability to clear required background check
PREFERRED QUALIFICATIONS
Bachelor's degree in area of Special Education, Social Services, or Education
Knowledge of transition services for special education

This position is virtual with some required travel for professional development and for testing.

The noise level at meeting locations throughout the stateistypically moderate and should not vary much from what is experienced in the home office.
