Job Description

Schedule: Part time. Wednesday, Thursday, and Fridays. 9:30am-6pm. Pay Range: $19.48 -$24.00, based on experience.

Main Function:

  • Support safe, efficient transitions of care by coordinating medication delivery processes, enhancing communication across the care team, and ensuring patients receive medications prior to discharge.
  • Assists in the nonprofessional aspects of preparing and distributing medications & performs the clerical duties of the dept.
  • All medication-associated duties are performed under the direct supervision of licensed personnel (Registered Pharmacists).

Key Responsibilities:

  • Medication Coordination & Processing — 40%
    • Coordinate the preparation and processing of discharge prescriptions in collaboration with our outpatient pharmacy.
    • Ensure discharge medications are prepared in alignment with patient flow and discharge timelines.
  • Communication With Nursing, Providers & Clinical Teams — 25%
    • Serve as a liaison to nursing, providers, case management, and other clinical stakeholders regarding medication readiness and discharge planning.
    • Communicate status updates, workflow barriers, and medication-related concerns in a timely manner.
    • Coordinate with pharmacists to resolve discrepancies or delays impacting the discharge process.
  • Patient Engagement & Pre-Discharge Outreach — 20%
    • Conduct pre-appointment and pre-discharge calls to assess medication needs and potential barriers.
    • Inform patients of the medication pickup or delivery process.
    • Provide supportive, patient-centered communication to enhance the discharge experience.
  • Outpatient Pharmacy Coordination — 10%
    • Communicate with outpatient pharmacies to troubleshoot insurance issues, clarify prescription details, and assist in ensuring medication availability.
    • Maintain workflow continuity across inpatient and outpatient departments to support medication readiness.
  • Administrative & Supportive Duties — 5%
    • Document activities, communications, and interventions in pharmacy systems and the electronic medical record.
    • Assist with quality-improvement projects, data collection, and departmental initiatives as needed.
    • Perform additional technical duties within scope, and under the direction of the Director(s) of Pharmacy to support transitions of care operations.

Qualifications:

  • High School education or equivalency required.
  • Working knowledge of Microsoft Word and Excel.
  • 1-2 years of Healthcare/pharmacy experience preferred.
  • Pharmacy Technician Certification & NYS license preferred.

Pay Range$19.48 -$24.00
