Transitions of Care Pharmacist

Full Time I Second Shift

Hospital Pharmacy I Lexington, KY

$15,000 Sign on Bonus for Qualified Individuals

Department of Pharmacy Services of Baptist Health Lexington provides pharmaceutical services to patients across the continuum of care. These services include, but are not limited to verification of medication orders, drug distribution, sterile product preparation, hazardous drug preparation, and utilization of automated dispensing cabinets.

  • Transitions of care pharmacists are responsible for target interdisciplinary programs and clinical services and ensure pharmaceutical care programs are appropriately integrated throughout BH system.
  • Participate in all necessary aspects of the medication-use system while supporting comprehensive and individualized pharmaceutical care in their assigned areas.
  • Serve as clinical resources and liaisons to other departments, hospital personnel, or external groups. In their role, they may conduct practice advancement projects as well as patient care quality and regulatory compliance initiatives designed to improve medication-use processes or pharmacy practice.
  • Works collaboratively with physicians and other members of the healthcare team
  • Communicates with a high level of skill to interact with physicians, RNs, patients, and other healthcare providers. Utilizes strong ability to solve problems.
  • Provides consultations concerning medication therapy daily.
  • Provides education to patients, physicians, nurses, and other healthcare team members as requested.
  • Initiates, documents, and participates in department activities that promote positive patient outcomes. These activities may include, but are not limited to: MUE, ADR reporting, resolving drug therapy questions, therapeutic drug monitoring, identifying drug interactions and allergies, direct contact with prescribers to clarify major problem orders, and non-formulary drug requests.
  • Involved with both the admission and discharge medication reconciliation process.
  • Collaborates with patients, families, and providers to ensure a safe continuation of care from the inpatient to the outpatient side of healthcare.
  • Provides patients and/or caregiver’s drug therapy and disease state counseling when needed.
  • Works with retail pharmacy to promote and enhance the Meds to Beds Program.
  • Clinically reviews and facilitates outpatient drug therapy to promote adherence and access post discharge.
  • Aid the social workers and case managers in obtaining prior authorizations or medication costs as time permits.
  • Serves as a pharmacy specialist for providers, nursing, and other staff on clinically complicated patient cases, investigation of adverse drug events and/or management of drug shortages as needed.

Requirements:

  • Bachelor’s Degree Pharmacy.
  • PGY-1 Residency Completion; preferred but not required.
  • Kentucky Board of Pharmacy license in good standing as applicable to practice site.
  • One year related work experience.
