The Transition Delivery Manager is responsible for coordinating and driving the successful delivery of client transitions from initiation through go-live and handover to Business as Usual (BAU). Working closely with the clients and cross-functional teams, the role ensures that all transition activities are planned, governed, monitored, and executed in line with agreed timelines, quality standards, and business objectives.

Role description and requirements:

• Lead the day-to-day, end-to-end delivery coordination of client transitions, working closely with the Transition Lead and all relevant workstreams.
• Create, maintain, and manage the integrated transition plan, including milestones, dependencies, critical path, deliverables, and resource requirements.
• Establish and coordinate the transition governance cadence, including workstream meetings, status reviews, SteerCo preparation, decision tracking, and escalation.
• Own the consolidated RAID log and ensure that risks, assumptions, issues, and dependencies are actively followed up, assigned, and escalated where required.
• Coordinate delivery across workstreams and other involved functions.
• Monitor transition readiness against agreed entry and exit criteria, ensuring that gaps and required mitigation actions are transparent.
• Prepare accurate and timely transition reporting for internal and client stakeholders, covering progress, milestones, risks, decisions, resources, and overall readiness.
• Coordinate the preparation, review, and completion of key transition deliverables, including process documentation, operating procedures, training materials, and handover documentation.
• Coordinate testing, training, cutover, go-live, and hypercare activities across the responsible workstreams.
• Ensure a structured handover into the BAU organization, including completion of agreed deliverables, acceptance criteria, open-action tracking, and governance closure.
• Build effective relationships with internal and client stakeholders and support clear, proactive communication throughout the transition.
• Apply the Chain IQ transition methodology, templates, governance, and quality standards, and contribute lessons learned to their continuous improvement.
• Support solution design, transition planning, effort estimation, and sales activities for new opportunities where required.
• Coach and support more junior transition resources where applicable.
• Travel internationally depending on project requirements.
