We are seeking to appoint a dynamic and motivated individual to the role of Transitions Keyworker. This is an exciting opportunity to join an expanding area of the curriculum, which boasts continued learner success and achievement.
The successful candidate must be able to ensure that students with an EHCP make a successful transition from schools and other post 16 provision, into College. This will involve working closely with other agencies, institutions and parents to design a transition plan for Supported Learning students and making sure they have a fully supported transition, into College.
