
Job Title: Transitions Coordinator – Senior Living Division
Status: Exempt
Reports To: Home Choice Manager
Purpose
As a Transitions Coordinator you would work Monday through Friday (no weekends) playing a vital role in helping adults move from long-term care facilities to home or community-based settings. Following organizational policies, state/federal regulations, and accreditation requirements, this position ensures a smooth, person-centered transition for each consumer, under the general supervision of the Home Choice Manager.
Essential Functions
Serve as a liaison between healthcare and housing professionals, including service coordinators, discharge planners, and community-based service providers. Maintain knowledge of all service lines: adult day health, personal care/private duty, service coordination, skilled care, and hospice.
Collaborate closely with individual case managers to arrange services that align with each consumer’s preferences and community resources, including physicians, pharmacies, and other essential providers.
Assist individuals in locating housing and obtaining necessary materials and supplies.
Support consumers in accessing entitled benefits and resources.
Provide exceptional customer service by addressing questions, concerns, and problems, ensuring daily needs are met.
Non-Essential Functions
Assist with the completion of consumer workbooks when needed.
Support participants with employment options if interest is indicated.

Maintain a professional, positive demeanor and manage time efficiently. Serve as a role model promoting teamwork and proactive problem-solving.
Ensure all client interactions uphold Client Rights, respect, and dignity. Report complaints, abuse, misappropriation of funds/property, or any corporate compliance issues immediately.
Adhere to all National Church Residences policies, including Employee Information Guide (EIG), facility handbooks, drug-free workplace, safe work practices, and all federal, state, and local regulations.
Perform other duties as assigned.
Qualifications
Education:
High school diploma or GED required.
Healthcare or social services experience preferred.
Experience:
Minimum of 1 year in social service delivery with elderly and/or family populations; 2–4 years preferred.
Knowledge of supportive services and community resources in assigned areas.
Travel:
This Hybrid role will travel frequent to visit clients across Montgomery, Greene, Clark, Preble, Darke, Miami, Shelby, Mercer, Auglaize, Champaign counties, and other assigned locations.
This role is paid for travel time and offers reimbursement for mileage.
Licensure:
Valid driver’s license and compliance with National Church Residences motor vehicle policy.
