Job Description

Shift: Monday - Friday 9AM-5PM

Community Medical Services (CMS) is hiring Transition Care Coordinator responsible for coordinating and overseeing client transitions within Community Medical Services and to external OTPs, in addition to linking clients with available resources, providing case management services, advocating for clients’ best interests, and maintaining accurate documentation. Above all, the Transition Care Coordinator is responsible for providing patient-centered care in a professional environment.

Responsibilities:

  • Coordinates care and transfers for clients transitioning to other CMS clinic locations and external OTPs.
  • Develops and maintains relationships with external OTPs and other community substance use disorder and behavioral health resources.
  • Advocates for clients with a variety of service providers.
  • Assists in collection of consent and related medical records.
  • Coordinates discharge planning
  • Maintains HIPAA and 42 CFR Part 2 confidentiality with all communications.
  • Completes necessary administrative tasks.
  • Maintains clear and concise documentation of transition progress for clients.

Requirements

  • High school diploma or GED required
  • Minimum of two years of experience working with individuals with substance use disorder
  • Three to five years of experience in a clinical or community resource setting with care coordinator and/or case management experience preferred
  • Ability to travel to assigned CMS sites
  • Basic to intermediate computer knowledge
  • Problem solving, conflict resolution, and strong customer service skills
  • Passion, humility, integrity, positive attitude, mission-driven, and self-directed
  • Excellent organizational skills, accuracy, and attention to detail
  • Ability to keep accurate notes and records
  • Culturally competent and sensitive to client and employee needs
  • Ability to communicate clearly and effectively between all organizational levels and with outside providers
  • Ability to match client needs with available community services
  • Knowledge of local, state, and federal regulatory requirements

Tools and Equipment Requirements:

  • The ability to use a phone, computer, printer, and copier is required.
  • Frequent use of Microsoft office products, including but not limited to Outlook, Word, Excel, and PowerPoint.
  • The ability to use the internet and various web browser software is required.

Physical Working Conditions and Office Setting Description:

  • Requires sitting and standing associated with a normal office environment.
  • Manual dexterity using a calculator and computer keyboard.
  • Requires prolonged sitting, standing, frequent bending, stooping, or stretching.
  • Frequent and prolonged typing and frequent and prolonged operation of computer, keyboard, and telephones required.
  • Requires occasional use of fax machines, telephones, copiers, and other office equipment.
  • Employee must regularly lift and/or move up to 10 pounds, frequently lift and/or move up to 25 pounds and occasionally lift/and or move up to 50 pounds.
