Perform
vital signs assessment and pain score prior and/or post exercises or activities whenever necessary
Provide
advice on fall prevention and home safety.
Discuss
recommendations for clients who require assistive devices under various support
scheme with Primary Therapist
Conduct
exercise sessions in adherence to the care plans prescribed by Primary Therapist
Conduct
maintenance activities which includes physical, recreational and other
activities planned by the Centre Manager with inputs from the Primary Therapist
Work with
care staff to surface issues at the Multi-Disciplinary team to meet the needs
of the respective patients
Monitor
clients response to treatment and report red flag issues and change in mental
status, side effects of medication, unusual incidents which impede on safety to
staff-in-charge in a timely manner
Coordinate
clients visit schedule using the weekly planner
Conduct
training to patient and caregivers
Perform
proper documentation of patients care/progress notes in a timely and accurate
manner
Collect
payment fee by checking the subsidized status of patients
Maintain
Inventory for supplies and equipment
Simple
equipment maintenance
Job Requirement
GCE O or
N level or WSQ Higher Certificate in
Healthcare Support (Therapy Support), Certificate in Therapy Services or
equivalent
Physical
fitness to lift and manoeuvre patients during treatments and exercise for
extended periods of time
Computer
literate to do electronic entry into electronic medical record.
Experience
in healthcare/community care setting would be an advantage.
Salary range: $1600
- $2010

to 5.30pm
