That's why we're bringing our innovative Home and Community Neuro Rehabilitation Program to California's Central Coast and building a team of passionate clinicians dedicated to helping individuals reclaim their independence where it matters most.

Serving clients throughout San Luis Obispo, Arroyo Grande, Pismo Beach, Grover Beach, Paso Robles, Templeton, and surrounding communities, our therapists empower individuals recovering from brain injuries, stroke, spinal cord injuries, Parkinson's disease, and other neurological conditions to achieve meaningful goals in their everyday lives.

This isn't traditional therapy. Our clinicians meet clients where life happens, helping them navigate real-world challenges and milestones. One day you may be working with a client in their kitchen as they prepare a meal independently. The next, you may help someone build confidence navigating a grocery store, returning to work, enjoying outdoor recreation, or reconnecting with the activities they love.

Whether it's hiking a favorite trail, visiting a vineyard, paddleboarding along the coast, shopping for groceries, attending a community event, or returning to meaningful employment, every therapy session is personalized, purposeful, and life changing.

Responsibilities

  • Performs comprehensive evaluations, assessing the needs of the patients, and sets measurable objectives and goals that are formulated in conjunction with the rehabilitation team
  • Adjusts treatment to achieve maximum results in independence and/or function
  • Plans, organizes, and coordinates recreation activities, functional leisure skills, and leisure education
  • Interviews the patient to assess capabilities/interests/barriers. Monitors progress and recommends changes/modifications to program as necessary
  • Designs community reintegration activities to assist the patient with their physical and cognitive needs during physical recreational/leisure activity Assists the patient in understanding their various needs and limitations and how to manage them during function
  • Regularly re-assesses effectiveness of treatment plan, progress of patient towards goals and the need for modifying goals or treatment
  • Communicates patient’s needs and progress to the treatment team, physician, patient, family members, and others as applicable
  • Observes/documents and reports changes in the condition/status of the person receiving services to the Clinical Coordinator, Program Manager or DCM, attending physician and other treatment team members
  • Consults with medical providers regarding patient evaluation & treatment, progress of the patient & discharge from treatment
  • Maintains and submits evaluation, treatment and billing documentation in the format and timelines required by the Company, facility, funding source, local, state and federal regulation
  • Adheres to standards of the Company's Code of Business Conduct, policies, HIPAA and Corporate Compliance programs and appropriate professional practice standards and applicable state/federal laws
  • May work with rehabilitation specialists in fulfilling the patient’s treatment plan through instruction in patient-related activities to be carried out by the rehabilitation specialist with supervision provided by the Therapeutic Recreation Specialist
  • Performs other duties as assigned

Qualifications

  • Bachelor’s degree in Therapeutic Recreation Therapy, or the equivalent
  • Maintains certification as a Certified Therapeutic Recreation Therapist (CTRT) through the National Council for Therapeutic Recreation Certification (NCTRC)
  • Maintains professional license and/or certification if required by the state in which the CTRS practices
  • Minimum of two years therapeutic therapy experience and therapeutic recreation program design
  • Experience working with patients with neurologic or orthopedic injuries or conditions
  • Demonstrates effective and professional interpersonal, verbal, and written communication skills
  • Able to work independently and part of a team
  • Demonstrates strong organization skills and attention to detail
  • Has reliable transportation and can travel to/from multiple assignments/locations on a daily basis
