
The WellFund Disability Coordinator builds working relationships with clients and patients, completes and follows the entire Social Security Disability process from the initial application to the final allowance or denial. The Disability Coordinator identifies and resolves barriers to the Social Security Disability allowance while supporting patients and their family during the process. The Disability Coordinator works as part of an extensive team, including; Patient Advocates, The WellFund On-Site Manager, Hospital Case Management, Hospital Business Office/ Revenue Cycle Employees, Hospital Patient Financial Counselors and various other clinical and non-clinical hospital staff. The Disability Coordinator exhibits superior customer service skills and provides prompt courteous service to patients in an organized and professional manner at all times.

The position of Disability Coordinator often includes difficult situations for the patient and their family. This may include terminal illness, traumatic injuries, and situations involving law enforcement. Disability Coordinators are required to ask patients very personal, uncomfortable and difficult questions regarding their health and history; possibly including criminal acts such as illicit drug use; as well as, detailed descriptions of situations involving patients who are the victims of crime while remaining non-judgmental and compassionate regarding their situation.
Responsibility
The Disability Coordinator is responsible for interviewing patients, completing, reviewing and analyzing disability claims throughout the claims process. This involves reviewing the patient’s medical, psychiatric, social and vocational histories in order to complete the Social Security Disability Claim. Disability Coordinators are responsible for contacting patients/claimants, Patient Advocates, Case Managers and Social Security Personnel in order to write detailed medical, vocational and social summaries for each patient/claimant. The Disability Coordinator works in a team concept, but takes calls, completes follow-up, interviews patients and builds the case file on his/her own.
Personal Work Relationships
The Disability Coordinator must deal with a variety of staff levels, conditions and circumstances throughout numerous hospital departments and government agencies continuously working with colleagues, team leads, supervisors and management staff.
