TEMPORARY POSITION

FLSA Status: Non-Exempt

Department: Case Management

Reports To: Manager, Case Management

GENERAL DESCRIPTION OF POSITION

The LVN supports regulatory compliance for SNP care coordination through targeted data collection, case review preparation, drafting individualized care plans (ICPs), and creating SBAR summaries for ICT meetings. The LVN works under the supervision of a Registered Nurse and follows established workflows to ensure timely, accurate completion. Productivity will be monitored daily.

ESSENTIAL DUTIES AND RESPONSIBILITIES

To perform this job successfully, an individual must be able to satisfactorily perform each essential duty listed below.

  1. Conduct focused data collection and basic assessments for assigned member cases.
  2. Review clinical records, medication lists, encounter history, and care plan status.
  3. Draft ICP components based on outreach and member needs, under RN guidance.
  4. Prepare SBAR summaries for RN review and finalization.
  5. Report any significant changes or red‑flag findings to the supervising RN.
  6. Maintain accurate, timely documentation in accordance with SNP requirements.
  7. Support care coordination outreach and follow‑up tasks within LVN scope.
  8. Productivity monitored daily.

REQUIREMENTS – Required (R)

The requirements listed below are representative of the knowledge, skill, and/or ability required or desired.

  1. Current California LVN license in good standing. (R)
  2. At least 1 year of experience in care coordination, ambulatory care, or managed care preferred.
  3. Experience with chart review and care plan implementation preferred
  4. Strong attention to detail and ability to follow workflows.
  5. Excellent documentation accuracy, and communication skills.

WORKING CONDITIONS

Duties are primarily performed in an office environment while sitting or standing at a desk. Incumbents are subject to regular contact with co-workers, managers, external partners, and vendors, in person, by telephone, and via work-related electronic communications.

PHYSICAL REQUIREMENTS

Incumbents must be able to perform the essential functions of this job, with or without reasonable accommodation:

  1. Mobility Requirements: regular bending at the waist, stooping, reaching overhead, above the shoulders and horizontally, to retrieve and store files and supplies and sit or stand for extended periods of time;
  2. Lifting Requirements: regularly lift and carry files, notebooks, and office supplies that may weigh up to 25 pounds;
  3. Visual Requirements: ability to read information in printed materials and on a computer screen; perform close-up work; clarity of vision is required at 20 inches or less;
  4. Dexterity Requirements: regular use of hands, wrists, and finger movements; ability to perform repetitive motion (keyboard); writing (note-taking); ability to operate a computer keyboard and other office equipment
  5. Hearing/Talking Requirements: ability to hear normal speech, hear and talk to exchange information in person and on telephone;
  6. Reasoning Requirements: ability to think and work effectively under pressure; ability to effectively serve customers; decision making, maintain a concentrated level of attention to information communicated in person and by telephone throughout a typical workday; attention to detail.

ENVIRONMENTAL CONDITIONS

General office conditions. May be exposed to moderate noise levels.
