About the Role

As the Support at Home Lead, you'll be responsible for the overall success of the Support at Home program, balancing client outcomes, people leadership, operational excellence and commercial performance. The role exists to embed new ways of working, build capability within the team, and ensure clients receive personalised, high-quality care that helps them live well at home. The position includes leadership of Care Partners and accountability for quality, compliance, workforce capability and sustainable program performance.

You'll work closely with operational leaders, clinicians and support teams to drive strong outcomes for clients, employees and the broader business.

What You'll Be Doing

  • Leading and coaching a team of Care Partners to deliver exceptional client outcomes
  • Championing and embedding the Support at Home model.
  • Driving a culture of accountability, learning and continuous improvement
  • Supporting complex client care planning, risk management and service optimisation
  • Overseeing budget utilisation, service growth and sustainable program performance
  • Building confidence and capability across the team in care planning, funding and client conversations
  • Ensuring high standards of quality, compliance and documentation
  • Partnering with stakeholders, providers and clinicians to create exceptional client experiences
  • Leading teams through change with clarity, confidence and care.

About You

We're looking for an experienced people leader who understands the balance between great care, strong operational performance and commercial sustainability.

You'll bring:

  • Leadership experience within aged care, home care or community services
  • Strong knowledge of the Support at Home program or Home Care Package environment
  • Experience leading teams through change and growth.
  • Confidence developing and coaching high-performing teams
  • Strong understanding of quality standards, compliance and risk management
  • Commercial acumen and the ability to manage funding utilisation and service performance
  • Excellent stakeholder engagement and communication skills
  • A collaborative, solutions-focused leadership style
  • Relevant qualifications in Nursing, Allied Health, Health, Community Services or a related discipline would be highly regarded
