Eliot’s Homelessness Services Division is seeking a dedicated Team Lead to join our Community Support Program (CSP-HI). Through outreach and case management, CSP-HI provides services to address the health related social needs of members who are experiencing homelessness and are frequent users of acute health MassHealth services, or are experiencing chronic homelessness. Eliot’s CSP-HI teams work collaboratively with community agencies, and federal and state providers to address the needs of unhoused populations in communities across the Commonwealth.
The ideal candidate is a dedicated and experienced behavioral health professional with a strong background in clinical service delivery and team supervision, particularly within underserved or housing-insecure populations.
