Job Description

The Connected Beginnings Team Leader is responsible for leading and coordinating the Connected Beginnings team that delivers health outcomes to Doon side and Mount Druitt Connected Beginnings Projects. This includes coordinating health care for birth to five-year-old children to facilitate better outcomes by the time they start school with a focus on the following domains:

  • physical health and wellbeing.
  • social competence.
  • emotional maturity.
  • language and cognitive skills.
  • Communication skills and general knowledge.

This project delivers the health component of the Connected Beginnings project that are lead by education agencies Ngroo, Mount Druitt and Ngalu Wal, Doonside.

The team leader would be responsible for facilitating a cohesive partnership between health and the education lead agencies and managing the staff on the Connected Beginnings Health Team.

Key Responsibilities but not limited to:

  • Mentor and role model the highest standard of work ethics.
  • Communicate effectively and negotiate with key stakeholders, families and community members.
  • Collect and report on data of participants of the Connected Beginnings Project
  • Report to Executive Management and the Board monthly and or as requested on progress and outcomes.
  • Deliver health screening, referral and follow up for children aged birth to 5 years.
  • Facilitate culturally appropriate healthcare for Aboriginal children living within Doon side and Mount Druitt regions.
  • Lead the team on health and safety practices.
  • Drive quality improvement processors and inclusion into the WACHS accreditation process.
  • Provide leadership to ensure that staff capability is developed to each team member’s full.
  • Assist in achieving the organization strategic objectives.
  • Implement an Indigenous succession plan for all non-identified positions.
  • Ensure that all employees develop and comply with work plans that are consistent with their funding agreements.
  • Ensure that all employees are complying with relevant WH&S legislation and that any issues are identified and resolved appropriately.

Experience, Qualifications & Licenses:

  • Experience in managing and supervising other health professionals.
  • Experience and capacity to facilitate partnerships and communicate effectively with key stakeholders.
  • Understanding and experience in the delivery of health care in a community setting for the health outcomes of children and families.
  • Excellent leadership, communication and interpersonal communication skills.
  • Effective organizational skills and the ability to manage competing priorities including time management.
  • Qualification of Registered Nurse or Endorsed Enrolled Nurse with current Registration with APHRA.
  • First Aid or willingness to obtain.
  • CPR or willingness to obtain.
  • A Certificate of Aboriginality
  • A Current National Police Check.
  • A Current Working With Children Check NSW.
  • Current Driver’s License that allows you to drive in Australia.
  • COVID-19 Vaccination and Booster information (or evidence of a valid medical
  • exemption).
  • Immunization records.
