POSITION SUMMARY
The Team Leader is responsible for the day-to-day operational oversight and management of employees who provide interdisciplinary services to patients and families assigned to their team. In order to ensure a high level of patient care and compliance, responsibilities will include clinical oversight, implementation and ongoing assessment of the hospice plan of care, monitoring patient/family satisfaction, quality improvement initiatives, compliance with state and federal regulations and standards, and ongoing training and evaluation of team members.
DUTIES AND RESPONSIBILITIES
- Ensures care and services are provided in a manner consistent with established standards of practice. Ensures care is individualized, optimizes the comfort and dignity of the patient and is consistent with patient/family needs and goals.
- Responsible for employing qualified staff and ensuring adequate orientation, education and evaluation of team members. Serves as mentor to team members. Makes field supervisory visits, pursuant to state and federal regulations, annually and as needed during the year to provide additional training, assess care delivery or monitor progress toward identified goals.
- Ensures that care delivered complies with agency policies and procedures and applicable federal and state hospice rules and regulations.
- Maintains fiscal responsibility by ensuring costs associated with patient care operations (including payroll, mileage, equipment, medications and supplies) are in line with budgetary constraints and organization policy. Ensures services provided are covered appropriately through the hospice benefit.
- Develops a culture of respect, compassion, teamwork and trust without losing sight of accountability to quality patient care.
- Works collaboratively with other Team Leaders and clinical leadership.

Qualifications
MINIMUM TRAINING, EDUCATION AND EXPERIENCE REQUIREMENTS:
- Graduate of an accredited school of nursing.
- Unrestricted Registered Nurse license in the state of Texas.
- One year operational and personnel management experience in a hospice or home health environment.
- CPR Certification.
- Valid Texas Driver’s license, auto liability insurance and safe driving record.
- Strong computer skills including knowledge of and experience with Microsoft Office products and experience with electronic medical records (EMR).
- Excellent critical thinking skills and solid nursing judgment.
- Demonstrated organizational, time management and detail-oriented skills.
- Ability to problem solve and be solution oriented in a timely manner.
- Ability to be self-directed and work independently in fulfilling role obligations.
- Strong interpersonal and communication skills.
- Ability to have crucial conversations with patients, families, and staff as needed.
- Ability to establish and maintain effective customer relationships, including healthcare partners.
- Ability to empathize and actively listen to patients, families, community and staff and work with team to ensure needs are met.
- Ability to hold staff accountable to ensure team is compliant with federal and state regulations.
PREFERRED QUALIFICATIONS:
- BSN preferred.
- Three years operational and personnel management experience preferred.
- Knowledge of hospice federal and state regulations and previous hospice or home health work experience preferred.
