The Case Management Team Lead works under the directions of the Manager and Director of Case Management. In addition to meeting the basic standards of Acute Case Management, the Team Leader is expected to role model and exceed staff standards of performance. This employee is to assume accountability for the quality of Case Management services delivered to our patient population. The Team Leader provides direct Case Management services, as needed, and oversees the services provided by other case managers and social workers in the department. The Team Leader assists management with the coordination and orientation of new staff members, contributes to the evaluation of new and seasoned staff performance, and provides continuing the education. In conjunction with the Manager, the Team Leader is also responsible for planning and managing the resources necessary for the patients we serve as well as the department. The Case Management Team Lead must be able to demonstrate knowledge of growth, and development to coordinate patient care appropriate to the age of patients served on their assigned clinical area.

Required: Certifications/Licenses/Experience

  • Active Registered Nurse (RN) in Indiana or a compact licensed state
  • Three (3) years of relevant clinical experience

Preferred: Certifications/Licenses/Experience

  • Case Management Certification
  • BSN or educational plan in place
  • Recent clinical experience in specific areas required for specialty clinical groups or clusters
  • Leadership Experience
