Job Description

Mending Point Behavioral Health, a program under The Methodist Home of Kentucky umbrella, is seeking a Targeted Case Manager (TCM) who is highly motivated to make a difference in the community and committed to the well-being of their clients. The TCM will utilize experience and knowledge to assess needs of clients and their families in order to link and coordinate the appropriate resources. They are responsible for managing an assigned caseload and responding appropriately to client needs in compliance with Medicaid standards. This includes but is not limited to assessing client needs; developing, implementing and reviewing service plans; and working with other community resources in achieving client service needs. Caseloads may include diverse populations with a variety of clinical needs. The Targeted Case Manager is a positive team player that promotes a culture of inclusion and participation. 

This is a new and developing position. We anticipate that we will continue to grow into several counties in the Northern Kentucky area.

Education & Professional Requirements:
  • Bachelor’s Degree from an accredited institution in the human services field with a minimum of 1 year of experience in a behavioral health setting post-bachelor’s degree.
  • Certification in the following Department for Behavioral Health, Developmental and Intellectual Disabilities approved trainings to meet the requirements outlined in 908 KAR 2:260:
    • Twelve (12) hour Targeted Case Management Core Training
    • Six (6) hour Substance Use Disorder Targeted Case Management Training
    • Six (6) hour Severe Emotional Disability Targeted Case Management Training
    • Six (6) hour Severe Mental Illness Targeted Case Management Training

Job Specifications
  • Must be at least 21 years of age.
  • Valid driver’s license, reliable transportation, and proof of current auto insurance.
  • Must submit to required background checks, fingerprinting and initial drug screen.
