Isaiah House is Kentucky’s largest non-profit, Christ-centered addiction recovery organization. We provide evidence-based, holistic care to individuals battling substance use disorders. With a heart for transforming lives, we serve individuals and families across the Commonwealth through a full continuum of care, from residential treatment to outpatient services and recovery support

We are seeking a compassionate and dedicated Targeted Case Manager (TCM) to support clients throughout their recovery journey by connecting them with the services and resources they need to succeed.

The TCM will coordinate the full continuum of case management services, including assessing client needs, developing individualized service plans, coordinating resources, monitoring progress, and evaluating outcomes. This position works closely with clients, treatment teams, community providers, and other resources to help address barriers and promote stability, independence, and long-term success.

The ideal candidate is organized, resourceful, and passionate about helping others navigate health and human services. Case management certification is a plus.

Job Duties:

  • Manage assigned caseloads, including assessing client needs, developing individualized service plans, monitoring progress, and determining appropriate case closure.
  • Assist clients in identifying and achieving goals that support recovery, wellness, stability, independence, and self-sufficiency.
  • Coordinate various aspects of client care, including referrals, community resources, appointments, and communication among service providers.
  • Advocate for clients and help them understand available services, resources, and treatment options so they can make informed decisions regarding their continued recovery and care.
  • Establish and maintain effective working relationships with clinical staff, medical providers, community agencies, and other members of the client’s care team.
  • Maintain consistent communication with clients to monitor progress, identify barriers, address changing needs, and provide ongoing support.
  • Connect clients with appropriate healthcare, housing, transportation, employment, benefits, educational, and community resources, as applicable to their individualized needs.
  • Maintain accurate, timely, and complete case documentation, service plans, progress notes, required forms, and reports in accordance with organizational and regulatory requirements.
  • Monitor the effectiveness of services and interventions and make appropriate adjustments to support positive client outcomes.
  • Maintain client confidentiality and comply with all applicable privacy, documentation, and professional standards.
  • Follow all organizational policies, procedures, protocols, and applicable federal, state, and regulatory requirements.
  • Perform other duties as assigned to support clients and the overall mission of Isaiah House.

Schedule: Monday–Friday, 8:00 AM–4:30 PM, with occasional overtime as needed based on client and operational needs.

Requirements

  • Previous professional experience in case management, behavioral health, mental health, healthcare, social services, nursing, or a related field preferred.
  • Strong understanding of case management principles, care coordination, community resources, and healthcare service delivery.
  • Knowledge of Medicaid requirements, reimbursement processes, and behavioral health services is a plus.
  • Excellent verbal and written communication skills, with the ability to communicate effectively with clients, families, treatment teams, and community providers.
  • Strong organizational and time-management skills with the ability to prioritize responsibilities, manage a caseload, and meet required deadlines.
  • Effective problem-solving skills and the ability to manage multiple priorities in a fast-paced environment.
  • Compassionate, client-focused approach with a demonstrated ability to work collaboratively as part of a multidisciplinary team.
  • Ability to maintain professionalism, appropriate boundaries, and confidentiality when working with clients and sensitive information.
  • Current license, certification, or registration, if required for the position.
  • Bachelor’s degree in a related behavioral health, social services, or healthcare field, with a minimum of one (1) year of post-graduate professional experience.
  • Employment with Isaiah House is contingent upon successful completion of all required pre-employment requirements, including a drug screening, background check, and Form I-9 employment eligibility verification. Failure to successfully complete required pre-employment screening may result in withdrawal of the employment offer.
