One of a network of provincial community health centres, the NorWest Community Health Centres is a community based, non-profit organization funded by Ontario Health, emphasizing access to primary health care and prevention programs and services.

NorWest Community Health Centres is seeking 1 FTE System Navigator for the Community Wellness Hubs to provide client-centered clinical care as part of an interdisciplinary team. The role supports individuals facing barriers to healthcare access, including those who are underhoused, living with mental health challenges, experiencing substance use, managing complex health conditions, and individuals who identify as Black, Indigenous, and People of Color (BIPOC), 2SLGBTQIA+, newcomers to Canada, or who are uninsured or underinsured. The successful candidate will be committed to health equity, cultural safety, and accessible care for underserved communities.

The primary focus of this position is to provide integrated, culturally responsive system navigation, advocacy, and care coordination services to Black youth and families within community settings, partner agencies, and designated outreach locations. The role promotes health equity, family empowerment, community engagement, and coordinated service delivery for Black communities experiencing complex health and social challenges.

The position has a competitive salary, the Health Care of Ontario Pension Plan (HOOPP) and an excellent group health benefit package.

The salary range for this position is $55,514 to $66,881 annually. Compensation will commensurate with qualifications and relevant experience.

QUALIFICATIONS

• Bachelor's degree in a related field (e.g. Social Work, Nursing, Health Sciences, Community Health).
• Two to three years’ experience in system navigation, health and social services, case management, community outreach or supporting youth and families in community-based settings.
• Ability to effectively engage clients in the assessment and planning with regards to their needs.
• Demonstrated ability to facilitate and advocate for services to support clients in their care.
• Ability to work independently while collaborating effectively with interdisciplinary teams, community partners, and service providers.
• Strong writing and documentation skills.
• Experience supporting individuals and families who face barriers to accessing care and services, including those related to race, gender identity, sexual orientation, income, housing instability, immigration status, disability, mental health, substance use, and systemic discrimination.
• Strong understanding of the social determinants of health and the ways systemic inequities impact outcomes for Black communities and other marginalized populations.
• Comprehensive knowledge of community resources, health and social service systems, and the ability to effectively connect clients to culturally relevant and accessible supports.
• Flexibility to work occasional evenings and weekends to meet the needs of youth, families, and community initiatives.
• Lived experience, community connections, and/or experience working within Black communities and with 2SLGBTQIA+ populations are considered strong assets.
• Valid driver's license, access to a reliable vehicle, and ability to travel throughout the community and to partner service locations.
